Here is the info you WILL need to gather in case of emergency:
Crime or Other Incident
- When (time, date) and where did it happen?
- Who (if known) was involved?
- What happened? What did the offending party do?
- Witnesses besides the reporter?
- When writing a report- Focus on FACTS not opinions (i.e. “I saw X do Y.”). Please be as specific as possible.
If this is a crime, you'll need to:
- Assess the situation. Do police need to be called? Does a supervisor need to be called? Does Operations need to be called?
- Assess the damage. Do we need emergency services?
- Take down names and contact information.
- Explain briefly what occurred:
“Unknown suspect was seen by myself and John Smith, breaking a window at the North Bay building on March 2nd, 2025 at 9:45am. The window is on the NE Corner of the building and is completely open and broken.” - Briefly describe the action you took at the time, or the action you think needs to be taken in future:
“After we called the police on said suspect, John Smith and I placed cardboard over the window and called Stacy Enriquez immediately.”
If need be, call your supervisor after emergency services. If not immediately necessary to speak to them, send an email.
If this is a facility-related incident, you'll need to:
- Assess the situation, Does a Supervisor need to be called? Does the Operations Director need to be called?
- Assess the damage. Do we need emergency services to come out, fire or police? (e.g. is the water leaking from a pipe?)
- Take down any witnesses name and contact information.
- Explain briefly what occurred.
- If need be, call your supervisor after emergency services. If not immediately necessary to speak to them, send an email.
Injury or Health Incident
- Name of people involved
- Brief description of incident
- Were paramedics called?
- Brief description of injuries
- Witnesses to incident, including contact information
When a report regarding an injury or another incident is needed:
- Assess the situation.
- Do EMTs need to be called? Does emergency contact of the injured party need to be called?
- If a possible head/neck/or back injury, DO NOT MOVE the person. Leave them exactly how you found them and call 9-1-1.
- If this is an instance of abuse, email info@northbay.cc directly. Include your name and details of the abuse. Your information will be kept confidential.
- Take down names and information of injured parties.
Phone number, home address, email address — whatever you can get. - Briefly explain what occurred.
Focus on facts not opinions. - Briefly describe the action that was taken at the time.
If necessary, call your supervisor, Lead Pastor or Operations Director.
